NOTE:
Applicants must pay a $760 filing fee to USCIS unless they qualify for a fee waiver. If you plan to
apply for a fee waiver, please ensure that you bring your most recent tax returns, Food Stamp or Medicaid Award Letter (in English), or Supplemental Security Income (SSI) Letter (in English).
- Green card (even if expired)
- All passports used in the last 5 years
- Home/school/employment history for the last five years (or three years if married to a U.S. citizen) Children’s information (date of birth, A#, current address)
- Marital history (Information about current and prior spouses including: dates of marriage, date of birth, and place of birth)
- If you have been arrested, cited or given a ticket, you must have your certificate of disposition and arrest record for each incident.
